Insurance and Pay Rates

Our insurance credentialing is currently pending with the following:  Medicare, Aetna, BCBS, Optum/United Healthcare.  We are currently accepting cash pay while insurance credentialing is in progress. 


Private Pay Rates

One hour Initial Evaluation Appointments are $300 

30 minute Follow Up Appointments are $175.  

Medication Refills

Medication refills may be initiated by contacting your pharmacy to refill the prescription via your pharmacy's automated refill system.  Please note that individuals requiring controlled substance prescriptions will be required to be seen in person for an initial visit and on a periodic basis.  Refills for any controlled substances must be initiated by contacting us at the office.  

Contact Information

We use the Spruce Health App to communicate with our clients via HIPAA-compliant healthcare communication.  Our phone number (940)340-3061 allows you to call us or leave a secure message for a call back.  Once we have your phone number, we can text you a secure link to join Spruce Health.


How to Connect with Our Office Using Spruce

  1. Click the secure link we texted to you.
  2. Download the Spruce Health app if prompted (it’s free).
  3. Open the app and enter your phone number.
  4. Enter the verification code sent to your phone.
  5. Once logged in, you’ll be automatically connected to our office.
  6. You can now send messages, complete forms, and receive updates securely.


Tips:

  • Make sure to enable notifications so you don’t miss messages.
  • If you already have the app, just click the link and it will open directly.

FAQs

Cancelled/Rescheduled Appointments and No Call/No Shows


We understand that circumstances may arise and require individuals to cancel or reschedule an appointment.  We also want to make sure that individuals that need an appointment are able to secure one.  For this reason, we require you to cancel or reschedule an appointment at least 24 hours notice prior to your scheduled appointment.  We do charge a $50 fee for the first no show or for any late cancellation appointment that is not cancelled prior to the 24 hour timeframe.  All other no shows/late cancellations will be assessed a $100 fee.  If you miss more than three appointments (no show or late cancellation) at any time during treatment, you can be discharged from the practice.  

Short Term Disability and FMLA paperwork

We do not complete any FMLA or disability paperwork on the first visit.  We want to partner with you to support your mental health and employment.  If you are currently seeking this paperwork, we encourage you to work with your current provider.  If you are an established client and we feel this paperwork may be beneficial, we can discuss it at that time.

NOTICE CONCERNING COMPLAINTS from Section 154.051 of the Occupations Code and TMB Rule §177.2

Complaints about physicians, as well as other licensees and registrants of the Texas Medical Board, including physician assistants, acupuncturists, surgical assistants, medical radiologic technologists, non-certified radiologic technicians, respiratory care practitioners, medical physicists, and perfusionists may be reported for investigation at the following address:
